Non-mydriatic digital macular photography: how good is the second eye photograph? Aung KZ, Robman L, Chong EW, English DR, Giles GG, Guymer RH. Centre for Eye Research Australia, University of Melbourne, Royal Victorian Eye & Ear Hospital, Melbourne, Australia. kzaung@unimelb.edu.au PURPOSE: In an elderly Australian population, to evaluate the quality of fundus photographs taken non-mydriatically in both eyes, and to compare the quality of those taken second with those taken first. METHODS: From 2258 participants (4516 images) aged 70 years and older who participated in the Melbourne Collaborative Cohort Study (MCCS), digital non-stereoscopic 45 degrees retinal photographs were taken with a Canon CR6-45NM Non-mydriatic Retinal Camera and evaluated. The quality of macular images was assessed as good, fair, and poor and McNemar's test was used to analyze variation in quality. RESULTS: Gradable quality images were obtained from 95.8% eyes of participants, with 93.9% of participants having gradable photos of both eyes. The gradable rate for the eye photographed first (right), was significantly higher than that for the eye photographed second (left): 89.7% vs. 85.6%, respectively (difference of 4.12%, confidence interval [CI] of 2.68-5.54%, p < 0.001). The rate of ungradable photographs from the second eye was slightly greater than the first eye (4.5% and 3.8%, respectively), but the difference in proportion was not statistically significant (difference of 3.6%, CI of 0.17-1.5%, p = 0.384). CONCLUSIONS: In the setting of a large elderly cohort study, non-dilated 45 degrees digital retinal imaging is an excellent method for fundus examination. It is fast, easy to use, non-invasive, and a reliable AMD (age-related macular degeneration)-detecting technique with only a minor loss of information from the second eye. Screening for sight threatening diabetic retinopathy using non-mydriatic retinal camera in a primary care setting: to dilate or not to dilate? Baeza M, Orozco-Beltrán D, Gil-Guillen VF, Pedrera V, Ribera MC, Pertusa S, Merino J. Clinical Medicine Department, Hospital Universitario Elche, University Miguel Hernandez, Alicante, Spain. OBJECTIVES: To evaluate three different strategies (without or with mydriasis) for community based screening for sight threatening diabetic retinopathy (STDR). DESIGN: A masked cross-sectional comparative study. SETTING: Primary care centre. PARTICIPANTS: 216 type-1 and type-2 diabetic patients (432 eyes). INTERVENTIONS: Screening for STDR was performed with the Topcon NW6S non-mydriatic camera (NMC), compared with the grading of the seven-field standard stereoscopic photographs, 30 degrees field photographs with mydriasis as the gold standard method. Three fields were studied: Field 1 centred on the macula; field 2 centred on the disc and the macula and field 3 centred on the disc, on the macula and temporal upper to the macula all without and with mydriasis. MAIN OUTCOME MEASURES: Sensitivity and specificity of screening method and prevalence of STDR. Strategies: field 1 (macular), addition of field 2 (nasal), addition of field 3 (superior temporal) all without and with mydriasis. RESULTS: The prevalence of STDR was 14.3%. When screening without mydriasis is performed, the percentage of referred patients increases from 14% (STDR) to almost 33% because of ungradable photographs. Sensitivity of detection of STDR by NMC using single field with mydriasis was 82% (95% confidence interval 72-92%) and without mydriasis 67% (54-80%). Specificity was 99% (97-100%) for single field with mydriasis and 99% (98-100%) without mydriasis. Sensitivity of detection using two fields with mydriasis was 95% (89-100%); without mydriasis (54-80%), slightly better than single field with mydriasis (72-92%). Specificity was similar. CONCLUSIONS: Screening for STDR with NMC can be performed effectively with one non-stereoscopic retinal photograph per eye with mydriasis using tropicamide 0.5%. Digital fundus image grading with the non-mydriatic Visucam(PRO NM) versus the FF450(plus) camera in diabetic retinopathy. Neubauer AS, Rothschuh A, Ulbig MW, Blum M. Department of Ophthalmology, Ludwig-Maximilians-University, Munich, Germany. aljoscha.neubauer@med.unimuenchen.de PURPOSE: Grading diabetic retinopathy in clinical trials is frequently based on 7-field stereo photography of the fundus in diagnostic mydriasis. In terms of image quality, the FF450(plus) camera (Carl Zeiss Meditec AG, Jena, Germany) defines a high-quality reference. The aim of the study was to investigate if the fully digital fundus camera Visucam(PRO NM) could serve as an alternative in clinical trials requiring 7-field stereo photography. METHODS: A total of 128 eyes of diabetes patients were enrolled in the randomized, controlled, prospective trial. Seven-field stereo photography was performed with the Visucam(PRO NM) and the FF450(plus) camera, in random order, both in diagnostic mydriasis. The resulting 256 image sets from the two camera systems were graded for retinopathy levels and image quality (on a scale of 1-5); both were anonymized and blinded to the image source. RESULTS: On FF450(plus) stereoscopic imaging, 20% of the patients had no or mild diabetic retinopathy (ETDRS level < or = 20) and 29% had no macular oedema. No patient had to be excluded as a result of image quality. Retinopathy level did not influence the quality of grading or of images. Excellent overall correspondence was obtained between the two fundus cameras regarding retinopathy levels (kappa 0.87) and macular oedema (kappa 0.80). In diagnostic mydriasis the image quality of the Visucam was graded slightly as better than that of the FF450(plus) (2.20 versus 2.41; p < 0.001), especially for pupils < 7 mm in mydriasis. CONCLUSIONS: The non-mydriatic Visucam(PRO NM) offers good image quality and is suitable as a more cost-efficient and easy-to-operate camera for applications and clinical trials requiring 7-field stereo photography. Screening for diabetic retinopathy by ophthalmologists and endocrinologists with pupillary dilation and a nonmydriatic digital camera. Deb-Joardar N, Germain N, Thuret G, Manoli P, Garcin AF, Millot L, Gavet Y, Estour B, Gain P. Department of Ophthalmology, Bellevue Hospital, Saint-Etienne, France. PURPOSE: To assess the effects of pupillary dilation on image quality and certitude of screening diabetic retinopathy by trained endocrinologists using a digital nonmydriatic camera. DESIGN: Prospective, comparative, observational case series. METHODS: One hundred fifty patients with diabetes attending a hospital-based photographic screening clinic had five-field (45 degree) digital retinal imaging and mosaic construction, first through dark-adapted pupils, then after a single application of tropicamide 1%. The 600 sets of images (150 patients, 300 eyes, before and after dilation) were graded independently and in a blinded manner by two endocrinologists and two ophthalmologists, with the consensual opinion on dilated images of the latter serving as the gold standard. The criteria of evaluation were assessment of image quality and certitude of detecting and grading retinopathy. RESULTS: Of 300 eyes, pharmacological mydriasis improved image quality, with an increase in the number of eyes with five good images from seven to 160 and good-quality mosaics from 99 to 233. Dilation allowed better identification of maculopathy in the second eye because there was a decrease in ungradeable central images from 127 to 15 eyes. Dilation increased the number of eyes having retinopathy detected with certainty from 153 to 252 and graded with certainty from 173 to 277. No adverse effects were recorded. CONCLUSIONS: Single application of tropicamide 1% significantly improves image quality and certitude of screening diabetic retinopathy with nonmydriatic camera by endocrinologists.
